Privacy notice: hypermobility enquiries and self-check
This notice covers the enquiry form on this page and the optional "Send my result" form on the hypermobility self-check (check.hypermobilitylondon.co.uk). It adds to our main Sutherland House privacy policy.
Who we are
Sutherland House Natural Health Centre, 1a Ashbourne Avenue, London NW11 0DP. Telephone 020 8458 7869. Email book@sutherlandhouse.life. We are the data controller. Our data protection officer is Judith Landhausser.
What we collect
- Enquiry form: your name, email, phone number, who the appointment is for (you or your child), the best time to call, and anything else you choose to tell us.
- Self-check "Send my result" form (optional): your name, email, optional phone number, and your check answers: your score, age group, whether you were checking for yourself or a child, and each answer.
- For both: the page you sent it from, the date and time, your browser type, and a scrambled (hashed) version of your internet address, used only to stop spam. We never store your full internet address.
The self-check itself saves nothing. Your answers stay in your browser unless you choose to send them using the form at the end.
Why we use it, and our legal basis
We use your details only to contact you about booking an assessment and to prepare for it. Information about your or your child's health is special category data. We only store it because you agree to it by ticking the box on the form (your explicit consent). You can withdraw that consent at any time by contacting us, and we will delete what you sent.
Children
If you are telling us about your child, please only send the form if you are their parent or guardian. We will talk to you, not your child, about booking.
Who can see it, and where it is stored
Only Sutherland House staff who handle bookings, and our website provider (Chris Matthews, RCM Solutions) for technical support. The forms are run on Cloudflare, which stores the data for us under a data processing agreement. The database that holds form submissions is in Cloudflare's European Union data region. The UK recognises the EU as giving personal data an adequate level of protection. We do not sell your data or share it for marketing.
How long we keep it
We keep enquiry details only for as long as we need them to reply to you and arrange an appointment. If you become a patient, your details move into your patient record and our main privacy policy applies. You can ask us to delete your enquiry at any time.
Your rights
You can ask to see, correct or delete your information, ask us to stop using it, or ask for a copy to take elsewhere. Email book@sutherlandhouse.life or write to us at the address above. If you are unhappy with how we have handled your information, you can complain to the Information Commissioner's Office (ico.org.uk).
